Q. Is there an "Industry Standard" cue sheet? A. Not really. PROs in the US, such as ASCAP and BMI, have established common standards in regards to terms and what information is required on a cue sheet for it to be valid. In addition, ASCAP and BMI have developed a common understanding of an industry standard, but each company has their own unique cue sheet form and an entirely different structure for submitting instances of music that is used in a commercial (i.e. tv or radio commercial). Also, there's many many other PROs throughout the world than just ASCAP and BMI, and each have their own requirements for the cue sheet paperwork and submission process.
